What Is Commonly Misdiagnosed as Pink Eye?

You wake up, glance in the mirror, and notice a red, irritated eye looking back at you. Your first thought might be pink eye, also known as conjunctivitis. While that’s a common assumption, our team at Eye Wellness knows red eyes can be a sign of many different things, and mistaking one condition for another can delay getting the relief you need.

Several eye conditions, from dry eye disease to more complex forms of inflammation, can resemble pink eye. A comprehensive eye exam helps you get the right treatment for your vision needs.

Signs of Pink Eye and Its Causes

What to Look For

Pink eye, also known as conjunctivitis, presents with a distinct set of symptoms depending on its cause. Bacterial and viral pink eye are the ones people are often concerned about because they are contagious, so it’s helpful to know what to look out for.

Signs that may be associated with bacterial or viral conjunctivitis include:

  • Redness in the white of the eye
  • A gritty or scratchy feeling, like sand is in your eye
  • Green or yellow discharge that can form a crust overnight (more common with bacterial)
  • Stringy, watery discharge (more common in viral)

If those symptoms don’t sound fully right, you may have a different kind of pink eye, or not pink eye at all. That’s why getting a professional eye exam can be so helpful, as the eye doctor can provide you with an accurate diagnosis and get you on the path to relief.

Common Types of Pink Eye

Not all pink eye is the same. The type you have determines the approach to care, as each one behaves differently and responds to different treatments:

  • Viral and bacterial: These types are contagious and often start in one eye before spreading to the other.
  • Allergic: This type is caused by allergens, so it isn’t contagious and often affects both eyes at the same time with intense itching.
  • Chemical: This type is not contagious and may develop after exposure to irritants such as smoke, air pollution, or chlorine.

Everyday Eye Issues That Can Be Confused for Pink Eye

Dry Eye Disease

If your eyes feel constantly irritated but don’t have a lot of discharge, you might be experiencing dry eye. This happens when your tears do not provide enough lubrication for your eyes and may cause symptoms such as:

  • A stinging or burning sensation
  • Redness and excessive watery tears
  • Moments of blurry vision that clear when you blink

Blepharitis

In some cases, the irritation may come from the eyelids rather than the eye itself. Blepharitis is an inflammation of the eyelids that can cause redness and irritation that may look similar to pink eye. You may notice symptoms such as:

  • Red, swollen, or itchy eyelids
  • Flaky, dandruff-like skin around the eyes
  • Crusty eyelashes when you wake up

A Stye on Your Eyelid

An eye stye is a small, tender bump that may form on the edge of your eyelid. This can happen when the base of an eyelash follicle or eyelid gland gets infected. While it can make the eye area red and swollen, the discomfort is usually focused in one spot rather than across the entire eye.

Common symptoms of a stye include:

  • A single, painful red lump on the eyelid edge
  • Swelling localized to one area
  • Discomfort or pain while blinking

More Serious Conditions That Can Seem Like Pink Eye

Keratitis

Keratitis is an inflammation of the cornea (the clear outer layer of your eye). Its symptoms can resemble a severe case of pink eye, but keratitis often involves more significant pain and vision changes.

It’s important to know that keratitis is considered an eye emergency because it can affect your long-term eye health if left untreated.

If you notice any of these symptoms, it’s a good idea to schedule an appointment or call our clinic right away:

  • Significant eye pain
  • Extreme sensitivity to light
  • Severe blurred or decreased vision

Uveitis

Uveitis is an inflammation of the middle layer of the eye, called the uvea. It can develop quickly and cause redness that might be confused with conjunctivitis, but the other symptoms are quite different.

While uveitis isn’t always an emergency, you should seek eye care if you notice:

  • A dull ache in or around your eye
  • Redness, often without much discharge
  • New floaters or blurry vision

If you are experiencing severe pain, light sensitivity, sudden vision changes, or persistent redness, it is important to seek care from your eye doctor for diagnostic testing. An eye exam can help determine the cause of your symptoms and get you the appropriate treatment as early as possible.

How to Spot the Differences and When to Get Help

Is It Pink Eye or Something Else?

Since many eye conditions share similar symptoms, it can be difficult to tell what may be causing your eye irritation. Paying attention to all your symptoms can provide helpful clues for your eye doctor.

  • Allergic conjunctivitis is often associated with intense itching, while viral or bacterial conjunctivitis may feel grittier, like sand is in the eye.
  • Pain or noticeable vision changes are usually a sign of something more serious
  • A stye is a distinct bump, not the all-over redness you see with pink eye

Why a Professional Diagnosis Is Important

Because many eye conditions share similar symptoms, self-diagnosing a red eye may lead you to seek the wrong treatment. While pink eye is common, redness, irritation, and discomfort can also point to other underlying conditions that may require different types of care.

During your eye exam, our eye doctors can use specialized tools to examine all parts of your eye, from the eyelid and cornea to the deeper internal structures. This detailed assessment can help identify the underlying cause of your symptoms and determine the most appropriate treatment for your situation.
